Identify Rubber Hose

Hey All,
The hose shown in the pic is cracked at both ends. I need help identifying it. Also, is this something I can get at a local auto parts store or do I need to go to the Dealer?

I believe that hose is part of the PCV.
i think it's 53013457AB

you can try to replace the hose but you'll need to make sure it's stiff enough that it wont collapse on vacuum.
