I left that part blank on my form and it didn't matter. I think that section is for heavy trucks, where the truck manufacturer and engine manufacture are different. i.e. International 9400 truck with Caterpillar C15 engine.
There's a great write up on this site to replace that hose without having to tear the front of the Jeep off. Rock Auto lists the high pressure hose for about $46 although only 1 in stock when I checked.
