You can seek out a Heavy Duty rear brake light bulb, they use them in over the road trucks, and Harley Davidson motorcycles because of the vibration factor.

I suggest that it is carbon buildup in the throttle intake body. Very little air is needed at idle/first start, the carbon buildup stops the air from getting through. Simple solution, old toothbrush & carb cleaner. Been there done that, seen it happen, seen the solution work. Worth a try...

Is the engine cold or warm/hot when it does on starting? Can you keep it running with the gas pedal?

Whine On Starting Common, often talked about subject. As already said it is the power steering pump loosening up the fluid. Reminds me of the days when diesel motors had small gas engines to start them, same noise. Whooupp.

Sounds like a disconnected/dry rotted/damaged vacuum line somewhere. Pop the hood, with engine running and listen, watch out for the fan blade(s).

If it is the resistor that is kaput, you as a rule will always get the fan to work on the #4 setting. I have not seen anybody say that they did NOT at least have #4. Hope that helps.

How about "CAML TOE"?

Did the rear gate window switch have an issue similar to this in this model year?

My guess is that the trans has a bad/going bad kickdown solenoid. The one that senses vehicle speed and kicks the gears down while coming to a stop.
