Here is some info copied directly from my FSM.
I hope this helps you.

BLEND DOOR ACTUATOR
DESCRIPTION
The blend door actuator is a reversible, 12-volt
Direct Current (DC), servo motor which mechanically
positions the blend door. The blend door actuator is
located on the top of the HVAChousing.
The blend door actuator is interchangeable with
the actuator for the mode doors. The blend door
actuator is contained within an identical black
molded plastic housing with an integral wire connector
receptacle. Three integral mounting tabs allow
the actuator to be secured with three screws to the
HVAC housing. The blend door actuator also has an
identical output shaft with splines that connects it to
the linkage that drives the blend air door. The blend
door actuator requires mechanical indexing to the
blend door linkage.
OPERATION
The blend door actuator is connected to the A/Cheater
control through the vehicle electrical system
by a dedicated three-wire lead and connector of the
instrument panel wire harness. The blend door
actuator can move the blend air door in two directions.
A potentiometer within the actuator allows the
A/C-heater control to know the exact position of the
blend door at all times.
The blend door actuator can be diagnosed using a
DRBIIIt scan tool. Refer to Body Diagnostic Procedures
for more information. The blend door actuator
cannot be adjusted or repaired and, if damaged or
faulty, it must be replaced.
REMOVAL
WARNING: ON VEHICLES EQUIPPED WITH AIRBAGS,
DISABLE THE AIRBAG SYSTEM BEFORE
ATTEMPTING ANY STEERING WHEEL, STEERING
COLUMN, OR INSTRUMENT PANEL COMPONENT
DIAGNOSIS OR SERVICE. DISCONNECT AND ISOLATE
THE BATTERY NEGATIVE (GROUND) CABLE,
THEN WAIT TWO MINUTES FOR THE AIRBAG SYSTEM
CAPACITOR TO DISCHARGE BEFORE PERFORMING
FURTHER DIAGNOSIS OR SERVICE. THIS
IS THE ONLY SURE WAY TO DISABLE THE AIRBAG
SYSTEM. FAILURE TO TAKE THE PROPER PRECAUTIONS
COULD RESULT IN AN ACCIDENTAL
AIRBAG DEPLOYMENT AND POSSIBLE PERSONAL
INJURY.
NOTE: With the ignition key in the on position put
the A/C-heater control in the mid-temperature position
and wait 10 seconds before turning the ignition
key to the off position. Then proceed to step 1.
(1) Disconnect and isolate the negative battery
cable.
(2) Disconnect the wire harness connector from the
blend door actuator by reaching through the glove
box (3) Remove the HVAC housing from vehicle (Refer
to 24 - HEATING & AIR CONDITIONING/DISTRIBUTION/
HVAC HOUSING - REMOVAL).
(4) Remove the screws that secure the blend door
actuator to the top of the HVAC housing. (Fig. 20).
(5) Remove the blend door actuator from the
HVAC housing. Note the actuator shaft position for
installation reference.
INSTALLATION
(1) Position the blend door actuator onto the
HVAC housing, making sure the actuator spline is
positioned properly with the door spline
(2) Install the screws that secure the blend door
actuator to the HVAC housing. Tighten the screws to
2.2 N·m (20 in. lbs.).
(3) Install the HVAC housing (Refer to 24 - HEATING
& AIR CONDITIONING/DISTRIBUTION/HVAC
HOUSING - INSTALLATION).
(4) Connect the wire harness connector to the
blend door actuator by reaching through the glove
box.
(5) Reconnect the negative battery cable

OK Fuse 30 also goes through "rear window fogger control" relay contact located in the BCM before driving the heat blender motor. Maybe the relay is bad.
I see the FSM states " The blend door actuator is interchangeable with the actuator for the mode doors etc." I am not sure what the "mode door actuator" is but maybe you can swap these two actuators over?...this seems to be what the FSM is saying!
The rest of the control comes through the "AC heater Control" and indirectly through the BCM. My circuit diagram is I believe incorrect (not unusual!) as I cannot see how the motor gets reversed so I cannot attempt to give you any more advice as regards measuring voltages etc.
Have fun!!
